引言 本研究旨在確定在全關節置換術（TJA）中，區域麻醉（RA）相較全身麻醉（GA）在死亡率和術后併發症方面是否具有優勢。
方法 這項基於人群的回顧性佇列研究納入韓國國家健康保險服務資料庫中2016-2021年間接受全膝關節或髖關節置換術且採用RA或GA的成年患者數據。 RA定義為脊麻、硬膜外麻醉或兩者聯合麻醉。 研究終點為30天死亡率、90天死亡率及術后併發症。 採用傾向性評分

結果 研究共納入517,960例TJA患者（RA組380,698例，GA組137,262例）。 經PS匹配后，最終分析包含186,590例患者（每組各93,295例）。 基於PS匹配佇列的邏輯回歸分析顯示，RA組相較GA組：
- 30天死亡率降低31%（OR： 0.69;95% CI 0.60-0.80;p<0.001）
- 90天死亡率降低22%（OR： 0.78;95% CI 0.72-0.85;p<0.001）
但兩組間總體術后併發症發生率差異無統計學意義（p=0.105）。
結論 在TJA手術中，區域麻醉相較全身麻醉與30天及90天生存率改善相關，但術后併發症發生率無顯著差異。 建議謹慎解讀本研究結果，並需更多設計嚴謹的前瞻性研究以明確最佳麻醉方案。
