一項由Unity Health、Sunnybrook Health Sciences Centre及多倫多大學研究人員開展的新研究發現，男性、黑人或西班牙裔糖尿病視網膜病變患者，以及居住地距離治療中心較遠者更可能錯過隨訪預約，面臨更高的視力喪失風險。
發表於《JAMA Network Open》的研究旨在制定保留患者持續治療的策略，以更好管理糖尿病視網膜病變——糖尿病患者視力下降的最常見誘因。 這種因高血糖損害視網膜導致的疾病影響著約四分之一的加拿大糖尿病患者，可能引發異常血管增生和糖尿病性黃斑水腫（視網膜血管滲漏液體）等併發症，最終導致視力損傷。
治療主要通過鐳射療法或抗VEGF（血管內皮生長因數）藥物注射阻止血管生長並減少視網膜滲漏。 "兩種治療都需要持續的治療和隨訪"，研究第一作者、多倫多大學Temerty醫學部三年級醫學生Ryan Huang表示。

網膜專家Marko Popovic的指導下，Huang與Sunnybrook Health Sciences Centre的醫學視網膜專家Radha Kohly及聖邁克爾醫院玻璃體視網膜外科醫生Rajeev Muni合作，分析了2012年1月至2021年12月期間2， 961名患者的電子醫療數據。 數據顯示17%患者在十年研究期內失訪，其中41%永久失訪，54%在一年後重返治療。
研究發現：男性或西班牙裔患者失訪概率高20-50%，黑人患者治療缺口風險是其他群體的兩倍; 居住地距離治療中心超過20公里的患者失訪風險隨距離增加而上升。 相反，基線視力較差或患有黃斑水腫的患者更可能堅持隨訪。 接受激光治療的患者比接受抗VEGF注射者更易失訪，但激光治療效果更持久。
研究團隊建議：對高風險患者優先採用鐳射治療，通過自動化簡訊提醒、定製文化語言教育資源、協調交通服務等措施改善隨訪依從性。 該研究對慢性疾病管理具有普遍參考價值。
