然而，非洲整體疫苗接種率仍低於全球標準：BCG疫苗接種率78%，麻疹疫苗68%，脊灰疫苗三針接種率70%，百白破疫苗三針接種率71%，均未達到全球組織80-90%的基準線。 令人擔憂的是，包括南非、埃及、剛果在內的12個國家雖非最貧困地區，但其疫苗覆蓋率增長速度不足以在2030年前達標。

接種率長期低於40%，而東非、南非平均達60%以上。 即便整體達標國家，其首都城市覆蓋率接近100%，但偏遠地區常低於50%。 - 經濟鴻溝擴大：在部分國家，富裕家庭兒童完整接種概率比貧困家庭高58%。 交通成本、信息獲取障礙和誤工成本構成主要障礙。
- 醫療系統脆弱：農村地區常存在一個護士服務數千人口的情況，疫苗斷供、冷鏈系統不完善、政治不穩定和文化偏見等問題持續影響接種進度。
研究特別指出，奈及利亞和安哥拉的貧富差距可能在2030年進一步擴大。 即便整體覆蓋率提升，仍將存在大量未接種兒童聚集區。 作者建議：
- 建立可靠的地方級監測系統精準定位未接種群體
- 加強基層醫療體系建設
- 優化疫苗供應鏈管理
- 實施針對性政策支持貧困人口
研究警示，未來五年將是關鍵視窗期。 非洲能否實現免疫目標不僅影響當前兒童健康，更將決定未來幾代人的健康韌性。 目前僅26個非洲國家有望實現全民健康覆蓋接種目標，其中馬拉威和布吉納法索是唯一兩個全境達標候選國。
